When my doctor ordered one up for me, I made the appointment and then spent hours trying to find out what the out of pocket cost would be after insurance kicked in. I was told that it “might” be in the $1,200 range (I could not get a firm answer), so I canceled the appointment until I could either find a better deal or budget for t
